This is a form for any lake association, volunteer, or professional to use when collecting water quality samples from a lake or aquatic system immediate to a lake. The form is a modified version of HETL’s form and has the addition of who should be emailed results. A form can be used for multiple samplings, but should be filled out as completely as possible for purposes with information regarding each sampling location and time. Any data sheets should be submitted to HETL and a copy should be retained by the individual collecting the sample or another designated representative in case of discrepancies.
